《1+2+1中美人才培养计划》学生合同（样表）
我已经和121项目协调员×××（或其他学校的教师/员工）讨论了《1+2+1中美人才培养计划》在美国乔治•梅森大学对所有学生的条款和要求。
我明白并同意以下条款：
• 我会按规定上课，我明白我不能超过规定的旷课次数。
• 我会按时上课。我明白上课迟到会在我的档案中做旷课记录。
• 我会尊敬我的老师和同学，并按照老师的要求去做；不会和其他同学交谈影响上课秩序。
• 我明白如果我的平均成绩达不到C, 能否完成121项目所要求的课程将受到质疑。
• 我明白如果学习成绩没有取得进步将会违反美国国际学生J-1签证的规定，这可能导致我失去签证。我会在×月×日把200×年××学期的进步状况报告给121项目协调员。
• 我明白这份合同的复印件会存档于1+2+1办公室，我也明白如果上述问题连续发生或不遵守上述规定，这份合同的复印件也会送到中教国际教育交流中心和我所在的中方大学。

STUDENT CONTRACT (SAMPLE)
US-China 1+2+1 Dual Degree Program
I have met with the 1+2+1 Program Coordinator XXX (and/or other university faculty/staff) to discuss the terms and conditions required for all students in the US-China 1+2+1 Dual Degree Program at George Mason University. In order to remain in the program, I understand and agree to the following conditions:
• I will attend all of my classes regularly. I understand that I may not exceed the number of absences allowed for my classes.
• I will come to class on time. I understand that coming to class late will result in absences against my record.
• I will treat my teachers and my fellow students with respect. I will do what the teacher asks and will not disrupt class by talking with other students.
• I understand that if I do not maintain a C average, it will jeopardize my ability to complete my required coursework in the 1+2+1 program.
• I understand that a failure to make progress in my studies is a violation of my status as an international student in the US on a J-1 visa and that this may result in loss of my visa status. I will report my midterm progress for the xx Semester 200x to the 1+2+1 Coordinator by the xxx date.
• I understand that a copy of this contract will be kept on file in the 1+2+1 office. I also understand that a copy of this contract may be sent to CCIEE and my Chinese University if the problems continue or if these conditions are not followed.
